At Tate Diagnostic we are dedicated to providing you with a pleasant visit. All of our services are performed by Registered Medical Diagnostic Sonographers. Arrive 15 minutes before your appointment. For convenience, complete your Tate Diagnostic New Patient Forms Packet (English) or Nuevo Paciente De Documentos (Español), print and bring them with you. Please bring the following to your appointment:
- Your Insurance Card(s)
- A Valid Picture ID
- Co-Payment (If Required)
- Completed and Signed Tate Diagnostic New Patient Forms Packet (English) or Nuevo Paciente De Documentos (Español)
We require 24 hour notice to cancel an appointment. We reserve the right to charge a fee for missed office visits. To cancel, change, or reschedule an appointment at our Manteca and Stockton Offices call 209.952.9300. To cancel, change, or reschedule an appointment at our Modesto Office call 209.236.1007.
Insurance and Billing
We are an in-network facility for most insurance companies. Contact our Billing Office at 209.952.9300 before your first appointment to ensure your visit is covered by your insurance. Call our Billing Office for any questions you have about your account.
